Does anyone have an action item log they are willing to share?

    Why not stick with What, Who, When (Start/Finish), Where, Why, How, and How Much (optional)? Tack on a Status column (Pending, In Progress, On Hold, Cancelled, Delayed, Complete) and a Comments column. Is that what you're looking for?

    Might not be exactly what you are looking for but I use this for tracking action items for many things.

    I was also thinking that an issues log (from the world of Project Management) might work.
