    Can someone help me with templates for supplier evaluation form and supplier reg. form suitable for service organisation(engineering)?

    Start by finding a template on the internet, and then modify so it meets your business needs.
    It is important that you evaluate the supplier for your needs e.g. do they need to be ISO 9001, ISO 14001, ISO/TS 16949 etc
    Just ask the information you need to know.

    As has been said make it fit your operations.

    Also keep it simple, the number of times that I have received questionnaires that are multiple pages long, and not easy to complete.

    Keep it as short as you can but still meeting your requirements.
