    Small company where management is actively involved in the day to day operations.

    Worse! I'd suggest it's like ending a soccer season, then looking at each game's score (only) and trying to figure out what went well - or wrong - to result in being #9 in the rankings...

    I'd suggest that while this has been my personal experience - in a shop of about 7 to 10 people - practically it means someone is NOT taking care of some vital aspect of the business, like sales/marketing, employee engagement/retention, improvements or similar. Being too tactical, means the strategy (planning) isn't taken care of.

    If "1 management system audit" means 1 full cycle audit of all QMS processes from audit planning to reporting to top management for their decisions and actions, then 1 full cycle per year is acceptable for an organization with frequent management/operational meetings (e.g. at least once per quarter). The audit of QMS processes should be staggered/distributed on all quarters of the year.

    1 audit a year. Hmmm. Does "2 audits a year" better? What about 10 audits a year? Tricky. Maybe the answer is "it depends..."

    The standard does a poor job in explaining what the purpose of an audit is, IMHO. Add to that the fact that the Certification Body Auditor corrupted things when asking for an "audit schedule". An audit programme shall be planned, taking into consideration the importance of the processes and changes. Those are not exactly predictable. Rarely in QMS-World do things go like clockwork, so why audit by time?
