   Good morning. I have a question about the frequency of the CQI-9 self-assessment. Does the term "annually" mean that the self-assessment should be carried out once in a given calendar year (January to December), or does it mean 12 consecutive months from the date of the previous self-assessment?

   Since it doesn't define what it means, do what you want. I'm not convinced by anything done once a year, however it's scheduled, but while people think it's "magical" (rather than "risk-based") we're stuck with it...
