documented information of management review

    Clause 9.9.3: The organization shall retain documented information as evidence of the results of management reviews.

    is the word Results here refer to management review outputs only or include inputs also

    i.e if the management discuss all inputs but inputs except internal audit

    so is this a NCR

    Results are just that, results. Inputs are considered and then the act of reviewing creates results. Who wrote an ncr and how was it worded?

    Management review is a process within an organization's QMS. A process, as illustrated in Figure 1 in the standard, has inputs and outputs (see below).

    ISO 9000 defines the word "output" as "result of a process". So, it follows that results here is about the "outputs of the management review process". These include decisions and actions related to a) opportunities for improvement, b) any need for changes to the QMS and c) resources needs (see 9.3.3).

    If your auditor is looking for "documented information as evidence of the results of management reviews", then he/she should be looking for records of "decisions and actions" as stated above. If the auditor insists that you must retain "documented information as evidence of the inputs covered", then you should ask: "Where in the standard does this requirement is specified?" If the auditor can't direct you to the exact words in the standard, then he/she is inventing a requirement and is not aware of the International Accreditation Forum's Auditing Practices Group Guidance for Evidence Collection. According to this guidance document (particularly for CB auditors): "Auditors should be aware that objective evidence does not necessarily depend on the existence of documented information, except where specifically mentioned in ISO 9001."
