    So they have added a requirement that does not exist to the standard. Not sure how kosher that is, but good to know.

    Exactly, Graham! Indeed, if you take the analogy of the frequency of sound(s) it is a mixture of frequencies. Anyone who tells you you have to have a "frequency plan" or anything like a calendar is deluding themselves as to what the standard actually says. Yes, audit programmes must be planned (not "calendared" or scheduled"), and that frequency must be defined. But not everything should be or indeed can be done to a predictable frequency. I'll bet that some CB auditors think that doing audits yearly, half yearly, monthly or something similar is the way to go, but they can't tell the difference between internal audits and external audits. The frequency will be defined for some aspects of the quality management system from the natural cadence of business - plus, as the standard DOES say: Importance of process(es) and in consideration of changes.
