A Scientific Approach to Determining Root Cause 2015-11-03

an introduction to an effective alternative to brainstorming to determien the causal mechanism.

  1. Bev D
An introduction to the use of 5-Whys in a disciplined and scientific manner to determine the causal mechanism of a Problem. The method is dubbed Y-X (read Y to X) and is a progressive and iterative strategy that starts with a sound Problem Statement and utilizes the principals of analytic studies and the "half-split" technique to disprove whole categories of factors to converge on the causal mechanism. A simple example is used to illustrate the concept and then two more complicated case studies are presented to demonstrate the versatility of the method.

The presentation focuses on technical problems rather people process based problems. It does not address MSA, Solution development, verification and validation or controls.
