    effectively? Please advice.

    What are you trying to accomplish, Raffy? Observations are anything an auditor sees - compliant or non-compliant. ISO 19011 defines them, I believe. What are opportunities for improvement? Something the auditor thinks needs improvement? Is it something non-compliant or not effective? If so, write a non-conformity statement.

    Don't be fooled by these "sheep in wolves clothing" type audit reports. Most management aren't interested in them
