    What You think about workplace instructions-> Is That preventive action?

    Yes work instructions are preventative...not very effective but they certainly can prevent some bad things from happening if people follow them.
    The list in the document you attached is pretty good. Mistake proofing (such as keying parts to prevent them from being mis-assembled), understanding the inputs effect on the outputs adn applying appropriate specifications and controls, SPC, etc. are all good preventative measures.

    Are you looking for more specific examples? If so it would be helpful to know what industry you are in....

    If you complete the PFMEA (correctly) with the resulting control plan(s) and instructions, that IS preventive action...

    Thank You for feedback. I know something more about preventive actions.

    I'm looking for due rates scale (S;O;D) for my industry.

    We produce heat exchangers (heating industry), so automotive scale is not suitable for us.

    Can You recomend something?

    Hi, I need your advice:

    We have an action:
    Measurement of first 3 produced pieces after tool adjustment in the beginning of the production batch (so called start-up release)

    Is it a preventive action because:
    we measured first pieces, detected the non-conformity then made a corrections and started to produce good parts. In other words with this measurement we prevented the production of bad parts.

    Or

    is it a detection action because:
    we already produced bad parts (first pieces of the batch are still the produced parts) and with this measurement we detected them

    Not really, you haven't prevented defects being produced. You entered into production without knowing (statistically speaking) what the process is capable of producing. If the defect occurred in the 4th, 5th and 6th parts, having signed off on the first 3, you wouldn't have prevented anything as you'd still have defects. You got lucky.

    Andy is right, it is detection only. Prevention is taking measures to not produce bad parts in the first place.

    Measurement of 3 Samples immediately after tool adjustment tends to "prevent" the occurrence of potential failure mode caused by "wrong tool adjustment". Another school of logic.

    In the definition of 'prevention' per the standard this is only preventive action if the 3 samples are not out of specification...the OP's scenario had the 3 parts of-of-specification.

    I get the point; Many times, it is about the Prevention of potential cause (Wrong tool adjustment in this case) as against the detection of potential cause; But I have also come across a methodology that considers Preventive control as the one intended to prevent the Potential failure mode caused by a potential cause factor; Set-up check after tool adjustment qualifies as a Preventive control in the latter case.

    Without statistical data, taking 3 of anything is going to be inaccurate as a "sample" to show prevention.
