Mohammad, do you mean 'process' or 'process map'?

Possibly that, as well...with the other two items being more 'documentation' in nature, I'm leaning towards process map, but hopefully Mohammad will come back and clarify. :)

    1. As Randy points out, ISO9000 tells us that a "quality manual" is "a specification (a "document stating requirements") for the quality management system of an organization". There are two problems with this:
    i) ISO9001:2008 listed (in 4.2.2) a number of items to be included in a manual, none of which was a "requirement for a QMS".
ii) ISO9001 states that the "quality management system requirements specified in this International Standard ..." - ie it is a "document stating requirements for a quality management system", which would imply that ISO9001 is a quality manual. Which it isn't. So the ISO9000 definition is flawed.
Fortunately, the requirement for a manual is no longer in the standard. In practice, most organisations nowadays have an electronic system which contains all relevant details of the system with suitable indexing to make all the elements readily accessible.

2 and 3. A process is a set of actions ("what you do to achieve an objective"). It is not a description, or a flowchart, or a diagram. And a procedure is a defined or accepted method to carry out an action or process.
