    Thanks for the recommendation of TapRoot Qualmx. It is an excellent approach for human behavior and equipment failure investigation. It is short of analytical tools for complex physics problems for ongoing failures. (but then again the tool for physics systems doesn't work well fro human behavior). The specific strategy, tactics and tools we select must fit the Problem type.
    The 'secrets' of effective investigations apply to almost any type of investigation tho and are well worth keeping them front and center in your mind.
